Naturopathy Services Agreement
This agreement is entered into between Seed Sync ("Practitioner") and the client ("Client"). This agreement outlines the terms and conditions under which the Practitioner will provide naturopathic services to the Client.
- Services Provided
The Practitioner agrees to provide naturopathic services, including consultations, dietary and lifestyle advice, herbal remedies, and other natural health recommendations, based on the Client’s unique needs. These services are holistic and personalized but are not intended to replace conventional medical treatment. The Client understands that naturopathic services focus on improving overall well-being and are intended to complement—not substitute—medical advice or treatment from licensed healthcare providers.
- Client Disclosure & Medical History
Client Responsibility: The Client agrees to provide complete and accurate information about their health history, medications, and any treatments they are undergoing. It is the Client’s responsibility to inform the Practitioner of any changes in their health status or medications during the course of their treatment.
Importance of Full Disclosure: Incomplete or inaccurate disclosure of medical history or current treatments may impact the effectiveness of naturopathic recommendations. The Practitioner will not be responsible for any adverse effects resulting from withheld information.
- Naturopathy is a Complementary Therapy
The Client acknowledges that naturopathy is a complementary and holistic approach to health and wellness. While it can be highly beneficial for improving quality of life, it should not be considered a substitute for diagnosis or treatment by a licensed medical professional. The Client is encouraged to maintain regular visits with their primary healthcare provider.
- Consent to Treatment
By signing this agreement, the Client gives their informed consent to undergo naturopathic treatment. The Client understands that they have the right to refuse or discontinue treatment at any time, and that all recommended treatments are voluntary. The Practitioner will outline the potential benefits and risks of each recommended treatment, and the Client agrees to participate with full awareness and consent.
- Booking Policy
Bookings for Naturopathic consultations at Seed Sync can be made online via our external booking system, Simple Clinic, accessible through the Bookings page on our website. Prepayment is required at the time of booking, and your payment and billing data will be securely collected by Simple Clinic.
You will receive a confirmation email once your booking is complete, and an SMS reminder will be sent 48 hours prior to your scheduled consultation.
- Pricing Policy
Pricing for naturopathic services may change without prior notice. Any price at the time of booking is final and will be charged accordingly. Discounted consultations or promotional rates are only available during specified promotional periods. All updated pricing will be reflected on our website.
- Client Intake Forms
New clients are required to complete the Initial Client Intake form at least 24 hours prior to their first consultation. This form will be sent via email after booking. If the form is not submitted before the consultation, it will need to be completed during the appointment, which will result in a reduced consultation time at the full consultation fee.
- Cancellation Policy
Seed Sync has a strict 48-hour cancellation policy. Cancellations made less than 48 hours before the scheduled consultation will incur 100% of the service fee.
Should you need to cancel, please contact us via the email provided in your booking confirmation or through Simple Clinic’s system. Failure to attend your appointment without prior notice will result in the full fee being charged.
- Rescheduling Appointments
You may reschedule your naturopathic consultation up to 24 hours before your appointment through Simple Clinic or by contacting us directly via email. Rescheduling requests made within 24 hours of the consultation will result in 100% of the service fee being charged.
In the rare event that the Practitioner is unable to attend due to illness or unforeseen circumstances, you will be notified as soon as possible, and you will be given the option to reschedule or receive a full refund.
- Consultation Etiquette & Behavior
We kindly request that all clients treat Seed Sync’s staff and practitioners with respect at all times. To ensure that your consultation runs smoothly, please complete the required intake forms 24 hours prior and arrive 5-10 minutes early. Late arrivals will not receive additional time, and the full service fee will still apply.
Seed Sync enforces a zero-tolerance policy for aggressive or inappropriate behavior. Any form of disrespectful or inappropriate language, gestures, or actions will result in the immediate termination of the consultation, and 100% of the service fee will be charged.
- Confidentiality and Data Security
Seed Sync is committed to protecting the confidentiality and privacy of your personal and health information. All information shared during consultations will be kept confidential and securely stored in compliance with data protection regulations. Health data and consultation notes will be managed within Simple Clinic’s platform, which adheres to the highest security standards to protect your sensitive information.
- Payment Security
All payments for naturopathic services are processed through Simple Clinic, a trusted third-party provider. Your financial and billing information will be securely processed in line with the highest industry standards to protect your sensitive data.
- Payment Terms
Fees for services are payable as outlined in the Practitioner’s fee schedule. Payments are due at the time of booking. If additional supplements, herbal remedies, or products are recommended, the cost of these items will be the responsibility of the Client.
- Follow-Up and Communication Between Sessions
The Practitioner will be available for brief follow-up communication via email between scheduled sessions to address any questions or concerns related to the Client’s treatment plan. More in-depth consultations will require scheduling a follow-up session. Response times to emails may vary but typically will be within 2-3 business days. For urgent health matters, the Client is advised to seek immediate assistance from their primary healthcare provider.
- Scope of Liability
The Practitioner will provide naturopathic care based on the best available evidence and professional judgment. However, the Client acknowledges that naturopathy is not an exact science, and individual results may vary. The Practitioner cannot guarantee specific outcomes or improvements in health. The Client assumes all responsibility for decisions made regarding their health and wellness, and the Practitioner is not liable for any adverse effects or lack of improvement resulting from the recommendations.
- Emergency Situations
The Client acknowledges that naturopathy is not intended for emergency care or urgent medical situations. If the Client experiences a medical emergency, they agree to seek immediate assistance from a licensed healthcare professional or call emergency services.
- Client’s Responsibilities for Success
The Client understands that achieving positive health outcomes requires personal responsibility. This includes actively participating in the recommended lifestyle changes, taking supplements or remedies as directed, attending scheduled appointments, and being open to discussing progress and challenges with the Practitioner. Success in naturopathic treatment is a shared effort between the Client and the Practitioner, and consistent engagement in the process is critical for effective results.
- Legal and Ethical Compliance
The Practitioner will adhere to all legal, ethical, and professional standards in the field of naturopathy. The Client agrees to respect the Practitioner’s guidelines and treatment plan. Both parties agree to work collaboratively in pursuit of the Client’s health and wellness goals.
- Termination of Agreement
Either party may terminate this agreement at any time, with or without cause, by providing written notice to the other party. Upon termination, the Client will be responsible for payment of all services rendered prior to termination. The Practitioner also reserves the right to terminate the agreement if the Client does not comply with the terms outlined, including but not limited to cancellation policies, payment requirements, or failure to follow treatment recommendations.
- Intellectual Property
The Practitioner reserves all rights to any intellectual property, such as handouts, guides, or other educational materials provided to the Client. The Client agrees not to reproduce, distribute, or share these materials without the Practitioner’s permission.
- Governing Law
This agreement shall be governed by and construed in accordance with the laws of NSW. Any disputes arising out of this agreement will be resolved in accordance with the laws of NSW.
- Agreement to Terms
By entering into this agreement, the Client acknowledges that they have read, understood, and agreed to the terms and conditions set forth herein. This agreement represents the entire understanding between the Client and the Practitioner and supersedes any prior discussions or agreements.
